MALR/GOVS on 19 January 2010 announced 17 household cases in 8 governorates (marked below with an asterisk * ). GOVS said the volume of cases was a result of the strengthening of passive surveillance in veterinary clinics. Veterinarians in the clinics were trained in a new HPAI case definition and use of a rapid field test. The veterinarians were given tasks for early detection of cases in the village, i.e., regular checks on the clinical status of birds in surrounding villages and testing for sick chickens in consistency with the case definition by using the rapid field test. In the cases announced January 19 (*), all samples taken by rapid field test were sent to the laboratory for confirmation by PCR technique regardless of the result of the rapid field test. The PDS team is responsible also for active surveillance in high-risk areas within household and following rumors on dead or sick poultry.

The affected flock was vaccinated one week before the sample collection, with a history of mortality before the vaccination. After vaccination all the chickens had died with a history of symptoms matched with the clinical case definition of HPAI.
On the day of the PDS visit, the PDS teams found Peking ducks in an apparently healthy condition except a few with blindness (a distinctive symptom in the HPAI clinical case definition in ducks).
During a return visit to the same village on 16 and 17 January 2010, vaccinated poultry at other households were found sick with symptoms matching with the clinical case definition of HPAI. According to GOVS instructions, no new samples have been sent to NLQP, and the response measures were implemented directly.

Comments: Source of the samples for confirmed case is dead and sick poultry thrown at the garbage collection area. In accordance with the PDS plan, teams targeted the high-risk areas in the village; among these are the garbage collection areas. The teams found many dead and sick poultry thrown at the garbage collection area.
--From interviews with families in the area, it was determined that one family bought ducks from a female street vendor. A day later, the duck developed nervous manifestations and yellowish diarrhea, followed by progressive poultry mortality in the existing poultry flock at the household. The family at the affected household thrown the dead and sick birds at the garbage collection area. The confirmed outbreak was reported in another house close to the garbage collection area, which has a scavenging poultry flock which eats remnants at the garbage site and thus was exposed to the sick and dead birds.
--A cat was found dead at the affected house which had a history of exposure to the infected birds. Samples were taken from the cat today for testing at NLQP.

Comments: The village is 500 meters from Lake Qaroun, and the domestic poultry has regular contact with migratory ducks. The outbreak started six months ago and was still circulating among the households around the time of sampling.
Three weeks ago, the household had sudden deaths with 100 percent mortality associated with cyanosis in comb and wattles for 200 layer chickens. After cleaning and decontamination, the household introduced new 40 young grower chickens bought from a nursery farm via peddler: Three days after introducing new chickens, mortalities started again with cyanosis in comb and wattles. The source of the outbreak is unknown. The newly purchased chickens may have been bought infected from the nursery farm, or may have contracted the infection at the household after contact with contaminated matter from the layer chickens that died in the earlier episode.

Comment:
The mortality started in a turkey provided as a gift from another family in the same village. The turkey died, and a duck developed nervous manifestations.
The layer chickens were vaccinated, were apparently healthy at the date of sampling, and were still alive with neither illness nor mortality for 3 successive days after sampling until the day of culling.
The poultry (pigeons and turkeys) at the household which provided the turkey as a gift had no problems and tested negative by PCR for HPAI.
The team concluded that the vaccinated chickens were previously infected and shedding the HPAI virus, infecting the ducks and newly introduced turkey. This reflects the risk of silent spread of HPAI unless there is an efficient surveillance program to monitor the virus circulation in vaccinated flocks.

--Families in the village have reported a history of massive spread of poultry deaths since March 2009 of unknown source.
--The village was the site of a confirmed human H5N1 infection (case number 72) in May 2009.
--In the current outbreak, the affected household experienced deaths among their poultry after they introduced new Peking ducklings (4 weeks old); the ducklings themselves appeared to be healthy on the date of sampling. The outbreak affected two neighboring houses only which used an open rooftop system.
--Part of the infected flock was vaccinated on May 2009: Among the vaccinated birds, the clinical signs were not severe and were limited to a decrease in egg production, ruffled feathers, and a few sudden deaths without any apparent clinical signs. Among the unvaccinated chickens (3 months old), mortality was high, with cyanosis in head and wattles.
